In the first instance, a physical illness or injury, such as a fall from a ladder resulting in chronic back pain, causes a mental illness such as depression. Typical features include episodes of repeated reliving of the trauma in intrusive memories ("flashbacks"), dreams or nightmares, occurring against the persisting background of a sense of "numbness" and emotional blunting, detachment from other people, unresponsiveness to surroundings, anhedonia, and avoidance of activities and situations reminiscent of the trauma.

Without diagnosis and proper treatment, PTSD or other mental illnesses can lead suffers to self-medicate with alcohol, illicit drugs, or prescription drug abuse. Men who have PTSD are 6.9 times as likely to develop depression and 10.4 times as likely to develop mania as men who do not have PTSD. 23 More than one half of men with PTSD also have a comorbid alcohol problem, and a significant portion of men and women who have PTSD have a comorbid illicit-substance use problem. 12 In patients who have PTSD, phobias tend to be more prevalent than generalized anxiety disorder or panic disorder; the risk of almost all anxiety disorders is increased markedly in these patients 23 ( Table 2 ). 12 The rate of attempted suicide in patients who have PTSD is estimated at 20 percent. 24 *—Data based on sample that included 139 men and 320 women with post-traumatic stress disorder.
